Absentee and Sample Ballots

You must be registered to vote before you can receive an absentee ballot. You can confirm your voter registration at https://myvote.wi.gov

Fill out the Absentee Ballot Request Form. Your town clerk must have a copy of your valid photo ID on file before sending you your absentee ballot. Detailed instructions for completion are on the back of the form.
