Power of Attorney

You will find statutory forms for Power of Attorney for Health Care, Power of Attorney for Finances, and Minor Child Power of Attorney (Delegation of Parental Authority) in this section.

These forms are provided as a courtesy.  The Register in Probate Office CANNOT assist you in preparing these forms, and we DO NOT file or record the forms once they are complete. 

A Power of Attorney terminates upon the death of the principal and DOES NOT give you the authority to act as the Personal Representative of an estate. 

Certain individuals may petition the Court to review the conduct of an agent acting under a Power of Attorney.  Such a petition is filed with the Register in Probate.  For specific information regarding this procedure, see Sec. 244.16 Wis. Stats.
